Spiritual Care Connect is an interactive program on spiritual health for patients and their families. It guides the user through understanding the connection between spirituality and their health, practical exercises to engage in, and opportunities to connect directly with a chaplain. There is also a special section for caregivers
Palliative Connect
Palliative Connect is a robust online telehealth program that revolutionizes the delivery of palliative care by assisting the palliative care team in educating and communicating with the patient and their caregivers. It serves as a tool to help the patient work with the palliative care team prepare for and engage in the important decisions that will determine the path of care. The program seeks to educate users on the elements of palliative care as well as guide the user through personalizing their palliative care. Palliative Connect also includes the support content found in Spiritual Care Connect.
